RISE WELLNESS CLUB

1. Eligibility & Activation

1. Memberships are available to new or existing patients. A Comprehensive Exam, Full-Series X-Rays, and Wellness Evaluation (“New Patient Visit”) must be completed within six (6) months of enrollment.
2. Clinical findings must indicate that routine preventive care is appropriate. Patients requiring specialty care or referral may be deemed ineligible.
3. The Sprouts™ plan may only be purchased for children ≤ 14 years in combination with an active Core™ or Thrive™ adult membership held by a parent or guardian. Sprouts™ memberships automatically upgrade to Core™ when the member turns 15 years old; the additional fee for the remainder of the term will be prorated.

Patients who re-enroll after a lapse in membership may be required to complete a new exam, updated records, or a revised care plan before membership benefits resume.

2. Term & Renewal

1. Each membership is valid for twelve (12) consecutive months beginning on the date the membership fee is paid (“Start Date”).
2. Memberships auto-renew annually at the then-current fee unless cancelled in writing at least 30 days before the renewal date.
3. Rise Dental reserves the right to change fees or benefits on thirty (30) days’ written notice before the next renewal.
4. Included visits, discounts, and any applicable Treatment Credit reset at renewal (i.e., at the start of each new 12-month membership cycle).
5. Membership benefits are designed to support consistent care over time. Preventive services are provided based on clinical need and recommended intervals and may not be used concurrently or in advance of appropriate timing. Preventive visits are typically spaced approximately every six (6) months or as clinically indicated. Benefits do not accelerate based on payment timing.

3. Fees & Payment

1. Annual membership fees are payable in full at activation. Monthly prices shown in marketing materials are for reference only.
2. A one-time $49 activation fee applies for each new member enrollment.
3. Payments are non-refundable except as expressly provided in Section 11 (Cancellation & Refunds).

4. Membership Tiers & Annual Fees

† Perio Maintenance Visits (PMVs) are only for patients who have completed a prescribed Scaling & Root Planing (SRP) procedure and achieved periodontal stability.

5. Preventive & Wellness Benefits

• All hygiene visits include Proactive Oral Cancer & Airway Screening.
• Members receive priority scheduling and extended appointment times.
• Core™ members enjoy Early Access to wellness workshops; Thrive™ members receive Priority Access.
• Sprouts™ members receive two complimentary Fluoride-Free Varnish applications per membership year, applied during their prophylaxis visits.
• Seasonal perks, workshops, and event benefits (if offered) are subject to availability and may change.

6. Treatment & Product Discounts

1. Discount percentages apply to Usual-&-Customary Fees at Rise Dental and cannot be combined with insurance benefits or other promotions.
2. Exclusions: Membership savings do not apply to laboratory-fabricated appliances, external laboratory fees, outsourced specialist services, prescription medications, products, or services provided by third-party providers.
3. If Treatment Credit is used (Section 7), it applies to eligible treatment completed during the active membership cycle, has no cash value, and cannot be applied to excluded items.

7. Limited Exams, Imaging & Treatment Credit

Limited Exams. Limited or problem-focused exams include all necessary X-Rays (PA or BWX) at no additional charge. Additional imaging (e.g., CBCT, Pano, Facial Scans) is discounted per Section 6.

Treatment Credit Option (in place of Limited Exams). Each membership includes two (2) Limited Exams per cycle. A member may choose to use the Treatment Credit option instead of using one or both Limited Exams during that same cycle.

• Earning rule: $100 credit per each $300 of eligible treatment completed.
• Maximum: Up to $200 in Treatment Credit per membership cycle.
• Cycle-based: Applies only within the current 12-month membership cycle and resets at renewal.
• Eligibility: Applies to eligible treatment performed by Rise Dental (excludes lab fees, outsourced services, products, and other exclusions in Section 6).
• No cash value: Not redeemable for cash and not transferable.
• Use limitation: If a Limited Exam is used, that portion of the Limited Exam / Treatment Credit benefit is considered used for the cycle.

8. Hygiene Visit Types & Upgrades

1. Routine Cleaning (Prophylaxis): For patients with healthy gums and no active periodontal disease. Includes removal of soft plaque, biofilm, and minor tartar above the gumline.
2. Perio Maintenance Visit (PMV): For patients who have completed Scaling & Root Planing (SRP) and are maintaining periodontal health. Includes localized subgingival cleaning, biofilm removal, and gum-health reassessment. PMVs are not “deep cleanings”; SRP is a separate therapeutic procedure performed before PMV eligibility.
3. Gentle Cleaning Upgrade (GBT): An optional enhancement using Guided Biofilm Therapy — a low-abrasion airflow powder, disclosing dye for visualization, and selective ultrasonic scaling. May be added to either a Routine Cleaning or PMV at member pricing.
4. Member-only pricing for wellness upgrades:
   • Gentle Cleaning Upgrade (GBT) — $49 per session (usable during hygiene visits)
   • Ozone Therapy Boost — $39 per session (usable during hygiene or restorative visits)

9. Exclusions, Limitations & Non-Transferability

1. Membership is non-transferable and applies solely to services rendered by Rise Dental.
2. Missed or same-day cancelled visits may forfeit that benefit for the membership period.
3. Products, event tickets, and seasonal perks are subject to availability.

10. Plan Changes

1. Upgrades: Members may upgrade to a higher tier at any time during their active term by paying the prorated difference; the membership term does not reset.
2. Downgrades: Members may only downgrade to a lower tier at renewal.
3. Sprouts Auto-Upgrade: Sprouts™ memberships automatically convert to Core™ upon the member’s 15th birthday unless the parent/guardian provides written notice to cancel at least 30 days before the birthday.

11. Cancellation & Refunds

1. Membership may be cancelled with thirty (30) days’ written notice prior to renewal.
2. Membership fees are non-refundable.
3. If membership is cancelled before the New Patient Visit is completed, any services received will revert to the full Usual and Customary fee, less amounts already paid, and the remaining balance will be due immediately.
4. If membership is terminated at any time after benefits have been used, services received may be recalculated at Rise Dental’s standard Usual and Customary fees, less amounts already paid, and any remaining balance will be due.

12. Coordination with Insurance

Membership discounts may not be combined with dental insurance, Medicare/Medicaid, or any other third-party benefit. Patients may choose at each visit whether to use insurance or membership pricing.

13. Modifications & Termination

Rise Dental may modify, suspend, or discontinue the Wellness Club, its fees, or its benefits at any time. Whenever possible, reasonable notice will be provided prior to implementation of material changes.

14. Governing Law & Dispute Resolution

This Agreement is governed by the laws of the State of Texas. Any dispute shall be resolved through binding arbitration in Williamson County, TX, pursuant to the American Arbitration Association’s healthcare rules.

15. Disclaimers

Not Insurance. This Membership is not dental insurance. It is a direct agreement with Rise Dental that provides access to certain preventive services and discounts. Members are responsible for payment of all dental services not specifically included.

Nature of Membership. This membership is not a prepaid bundle of services. It is a structured care plan that provides access to preventive services and benefits over a 12-month period.

Provider Participation. This membership applies only to services provided by Rise Dental. Specialty services, outsourced laboratory fees, and third-party providers are excluded.

Clinical Judgment. All treatment recommendations remain at the discretion of the treating dentist based on clinical findings.
