For patients 14 and under (with a parent member).

Membership is not dental insurance. Benefits are non transferable and must be used within the membership year. Membership is paid annualy and activates after your comprehensive new patient visit, and is valid for 12 months after activation. A $49 activation fee applies.

Community Support
Teachers, first responders, and military families are welcome to join the Rise Wellness Club at the same membership rates, with the option to pay monthly after committing to the annual program.
