Monthly Membership Enrollment Agreement

Effective February 23, 2026

This Monthly Membership Enrollment Agreement (“Agreement”) is incorporated into and made part of the Rise Dental Membership Terms & Conditions (“Base Terms”). All provisions of the Base Terms remain in effect unless specifically modified herein. In the event of any conflict, this Agreement controls as to monthly billing and payment structure.

1. Membership Term

  1. Enrollment in a Monthly Membership requires a minimum commitment of twelve (12) consecutive months beginning on the date of the first successful payment.
  2. Membership automatically renews annually unless cancelled in writing at least thirty (30) days prior to the renewal date.
  3. Included visits, discounts, and Treatment Credit reset at the start of each new 12-month membership cycle.

2. Monthly Membership Fees

  1. Sprouts™ Membership – $39 per month
  2. Core™ Membership – $59 per month
  3. Thrive™ Membership – $99 per month
  4. Monthly pricing reflects the annual membership fee divided over twelve (12) months. Membership benefits are identical to annual prepaid memberships.

3. Payment Authorization & Card-On-File

  1. Enrollment requires a valid credit or debit card on file and signed recurring auto-draft authorization.
  2. By enrolling, Member authorizes Rise Dental to automatically charge the card on file each month through its PCI-compliant payment processor (Clearent).
  3. Member agrees to maintain a valid payment method at all times and to update billing information promptly if changes occur.

4. Failed Payments & Suspension

  1. If a monthly payment fails, Rise Dental may reattempt the charge.
  2. A $35 failed payment fee may apply to declined transactions.
  3. After three (3) failed payment attempts, membership benefits will be suspended until the account is brought current.
  4. Preventive visits and member discounts may not be scheduled or applied while the account is delinquent.

5. Early Cancellation

  1. If membership is cancelled before completion of the 12-month term, the full retail value of services received will be calculated.
  2. Any outstanding balance between retail value and payments made becomes immediately due.
  3. No refunds will be issued after benefits have been used.

6. Eligibility Verification (If Applicable)

If enrolling under eligibility-based qualification (military, veterans, first responders, teachers, or approved special circumstances), documentation must be provided at enrollment. Verbal confirmation alone is not sufficient.

7. Non-Transferability

Membership benefits are non-transferable and valid only for the enrolled individual.

8. Governing Law

This Agreement is governed by the laws of the State of Texas. Any dispute shall be resolved through binding arbitration in Williamson County, TX.

9. Disclaimers

Not Insurance. This membership is not dental insurance. It is a direct agreement with Rise Dental providing access to certain preventive services and discounts. Members remain responsible for payment of all services not specifically included.
